ELIGIBILITY REQUIREMENTS

An Applicant Must:

Be a graduate of an accredited osteopathic medical school
Start up or Join a practice located in northwestern Indiana/southwestern Michigan area to practice osteopathic medicine ("area" is defined as being within a 60-mile radius of South Bend, Indiana ... Indiana counties include: St. Joseph, Marshall, Elkhart, Kosciusko, Starke and LaPorte. Michigan counties include: Berrien, Cass and St. Joseph)
Must remain in the Michiana area for two years
Serve 1 year on the OMF Board of Directors

An application form (indicating interest in the grant program, indicating short/long term goals for your practice of medicine in the Michiana area and request for financial assistance) must be submitted by November 1st for the board's consideration.
