ELIGIBILITY REQUIREMENTS

An Applicant Must:

• Be a graduate of an accredited osteopathic medical school
• Start up or Join a practice located in northwestern Indiana/southwestern Michigan area to practice osteopathic medicine ("area" is defined as being within a 60-mile radius of South Bend, Indiana ... Indiana counties include: St. Joseph, Marshall, Elkhart, Kosciusko, Starke and LaPorte. Michigan counties include: Berrien, Cass and St. joseph)

A letter (indicating interest in the program, plans for setting up practice and request for financial assistance) must be submitted by July 1st for the board's consideration.
